Stock Invoice, Statement and Work Ticket Forms
Present a professional image with our laser style (for laser or inkjet printers) or continuous NCR style forms
(for dot matrix printers). We stock a variety of economical forms, available in all white or a process blue
screen, with perforations that turn an 8 1/2"x11" sheet into two copies. Some forms are designed to tear into
four pieces for multiple uses. Just tell Lab Manager which form you are using, load the forms in your printer
and print. It's as simple as that!
Our double window envelopes paired with our stock forms save you time by eliminating the tiresome chore of
addressing envelopes. Your name and address is displayed in the upper window and your client's name and
address is shown in the second window. Stock form/envelope orders placed before Noon (AZ time) are usually

Scan your logo (including the address) into Lab Manager and it will print your logo on our stock invoice and
statement forms.
Usage is defined as:
I - Invoice, I/QC - Invoice+Quality Control, S - Statement, W - Work Ticket.
Laser Style Forms (Use in laser or inkjet printers)
This form has a perforation down the center creating 2 identical invoices or statements or 2 sequential work tickets.

Envelopes
All envelopes come with security tint. Our stock envelopes are designed to be used
with our stock forms. Your name and address is displayed in the upper left-hand
